About the Opportunity

What You’ll Do

  • Maintain ongoing caseload using evidence-based approaches.
  • Use relationship-based strategies for social support navigation.
  • Conduct telephonic/SMS outreach.
  • Develop Health Action Plan/Shared Care Plan.
  • Support nurse care manager with delegated tasks.
  • Collaborate on care issues; participate in case reviews.
  • Meet monthly encounter metrics.
  • Assist individuals securing community supports; coordinate physical care appointments.
  • Use online platforms for collaboration.

What You’ll Need

  • Minimum 1 year general work experience; case management preferred.
  • Located in California; reliable transportation or quiet HIPAA-compliant space for virtual work.
  • Bilingual English/Spanish required.
  • Strong cultural fluency & empathy.
  • Collaborative team player; organizational skills; technical proficiency (CRM databases,...).
  • Valid driver’s license & auto liability insurance.

Preferred Qualifications

  • Experience with complex chronic needs/homelessness/SMI/SUD.
  • At least 2 years case management experience.
  • Motivational interviewing skills.
  • Medical terminology knowledge.
  • Problem-solving mindset; detail-oriented multitasker.

Compensation & Benefits

Salary $22-$25/hour (~$45k-$52k annually) Health/vision/dental insurance; 401k plan; $100 monthly WFH stipend; gas reimbursements; flexible vacation; equipment provided; career growth opportunities.
